Psidium guajava L. (guava) is a versatile medicinal plant with significant therapeutic potential, particularly for managing inflammatory conditions. Its rich phytochemical profile—comprising flavonoids (quercetin, kaempferol), phenolic acids (gallic, ellagic), meroterpenoids (psiguajadials), and essential oils (β-caryophyllene)—exhibits multi-targeted anti-inflammatory effects by modulating NF-κB, MAPK, and NLRP3 pathways, inhibiting pro-inflammatory cytokines, suppressing COX-2 and iNOS, and enhancing antioxidant defenses. Ethnomedicinal uses for gastrointestinal and skin disorders are supported by pharmacological evidence. However, challenges such as phytochemical variability, poor bioavailability of key compounds, and safety concerns hinder clinical translation.
