Psidium guajava
L. (guava) is a versatile medicinal plant with significant therapeutic potential, particularly for managing inflammatory conditions. Its rich phytochemical profile—comprising flavonoids (quercetin, kaempferol), phenolic acids (gallic, ellagic), meroterpenoids (psiguajadials), and essential oils (β-caryophyllene)—exhibits multi-targeted anti-inflammatory effects by modulating NF-κB, MAPK, and NLRP3 pathways, inhibiting pro-inflammatory cytokines, suppressing COX-2 and iNOS, and enhancing antioxidant defenses. Ethnomedicinal uses for gastrointestinal and skin disorders are supported by pharmacological evidence. However, challenges such as phytochemical variability, poor bioavailability of key compounds, and safety concerns hinder clinical translation. Advances in analytical profiling (HPLC, GC-MS, LC-MS, NMR) and emerging delivery systems (nanoemulsions, microencapsulation) show promise in overcoming these barriers, though scalability and standardization remain issues. Future research should focus on standardized extracts with quantified bioactive markers, rigorous clinical trials, sustainable green extraction, and integration of omics approaches for mechanistic insights. Collaborative efforts between researchers, industry, and regulators are vital to establish quality standards, enabling guava’s transition from traditional remedy to clinically validated anti-inflammatory therapy with potential for functional food and nutraceutical applications.
